1. Go to this website -> Then look for the form to be completed online.

2. You can also print the form, fill it in and mail it (the address will appear on the form).

3. Registration by telephone is also possible at the following number: 1-833-486-3007 from 8:30 a.m. to 4:30 p.m., Monday to Friday.

4. Visit your nearest Service Canada office to apply in person.

5. For the hearing-impaired: Teletypewriter (TTY) 1-833-467-2700
